Blood Product Transfusion — Utah 2025 Protocol

NEW 2025 — Paramedic prehospital blood transfusion for hemorrhagic/traumatic shock.

Steps by Cert Level

PARAMEDIC: Insert 2 large-bore IVs/IOs. Initiate blood product transfusion (pRBC, FFP, or whole blood) per agency protocol.
PARAMEDIC: Target SBP >90 WITHOUT closed head injury. Target SBP >110 WITH closed head injury.
PARAMEDIC: No blood products available → 500mL NS boluses to BP targets. Once target met → saline lock, no further fluid unless BP drops.
PARAMEDIC: If hypotensive after 2L crystalloid → contact medical control.
