Airway Management — Utah 2025 Protocol

Stepwise airway management from basic maneuvers to advanced devices based on cert level.

Steps by Cert Level

ALL: Basic airway maneuvers — jaw thrust, head-tilt chin-lift, OPA/NPA. Suction as needed. Supplemental O2. Target SpO2 90–94%. ETCO2 monitoring when available.
EMT: BVM ventilation. Supraglottic airway (SGA) insertion (King LT or iGel).
AEMT: All EMT skills plus IV access. May use SGA devices.
PARAMEDIC: Endotracheal intubation (ETI). Surgical airway (cricothyrotomy) if failed airway. RSI if authorized. Waveform capnography MANDATORY for all advanced airways. Confirm with ETCO2 + auscultation. If SpO2 doesn't improve with BVM → advance to next airway level. Max 3 ETI attempts → surgical airway.
