Spinal Motion Restriction — Utah 2025 Protocol

Selective SMR only — routine backboarding is outdated. Penetrating trauma: no SMR.

Steps by Cert Level

ALL: Apply SMR if ANY: AMS (GCS <15, intoxication), midline spinal tenderness, focal neuro deficit, dangerous mechanism (diving, axial loading, high-speed MVC, fall >3x height), spinal deformity.
ALL: DO NOT apply routine SMR to all trauma patients — evidence shows harm.
ALL: Cervical collar for all with SMR criteria. Long board for EXTRICATION only — not transport. Use vacuum mattress or scoop stretcher for transport.
