General Trauma Management — Utah 2025 Protocol

XABCDE — stop bleeding BEFORE airway. Scene time <10 minutes for critical trauma.

Steps by Cert Level

ALL: X — eXsanguinating hemorrhage FIRST: tourniquet or wound packing BEFORE airway work.
ALL: A — Airway (manual maneuvers, C-spine). B — Breathing (tension pneumo, open chest wound). C — Circulation (IV, fluids, hemorrhage control).
ALL: D — Disability (GCS, pupils). E — Expose/Environment (full body exam, prevent hypothermia).
ALL: HEMORRHAGE: Direct pressure → tourniquet (extremity, 2–3in proximal to wound) → wound packing with hemostatic gauze for junctional wounds.
PARAMEDIC: Fluid resuscitation: 500mL NS boluses to targets. Without head injury SBP >90. With head injury SBP >110. TXA 1g IV over 10min if <3hrs from injury.
