Recognize life-threatening OB emergencies: eclampsia, placenta previa, cord prolapse, breech.
Steps by Cert Level
ALL: Left lateral decubitus position for all OB patients — relieves aortocaval compression. High-flow O2. Rapid transport.
EMT: Cord prolapse: do NOT push cord back. Elevate presenting part manually off cord. Transport emergent.
AEMT: IV access. Fluid resuscitation for hemorrhage. Monitor fetal heart tones if able.
PARAMEDIC: Eclampsia (seizure in pregnancy): Mag Sulfate 4–6g IV over 15–20 min. Diazepam or Midazolam if Mg unavailable. Calcium Gluconate at bedside as Mg antidote. Breech: do not attempt to pull — support buttocks, allow spontaneous delivery.
