Post Cardiac Arrest (ROSC) — Utah 2025 Protocol

ROSC is not the finish line. 12-lead, secure airway, avoid hyperoxia and hyperventilation.

Steps by Cert Level

ALL: ROSC confirmed: sustained organized rhythm + palpable pulse + ETCO2 rise >35mmHg.
ALL: 12-lead EKG immediately — identify STEMI. Activate cath lab if STEMI present regardless of neuro status.
PARAMEDIC: Secure airway. Do NOT hyperventilate — ETCO2 target 35–45mmHg, RR 10–12/min. SpO2 target 94–99% — avoid hyperoxia.
PARAMEDIC: Target SBP >90mmHg. Treat hypotension with 500mL bolus then vasopressors. Sedate intubated patients per Post-ROSC Sedation protocol.
